Job Description

We are seeking a compassionate and experienced Case Manager Level II to join our team. In this role, you will be responsible for providing post-release services to children and their sponsors. You will bring expertise in trauma-informed care, child welfare practices, and case management, as well as your ability to work with diverse populations. Your role will involve collaborating with local social services, community outreach programs, and resource providers to ensure children and sponsors receive the support they need.

What You'll Do
Manage an assigned caseload of post-release cases, ensuring compliance with all relevant policies and procedures.
Conduct thorough assessments, home visits (virtual and in-person), and work closely with children and sponsors to identify needs and provide support.
Collaborate with care providers and other stakeholders to verify and assess potential sponsors' ability to provide a safe and nurturing environment for children.
Utilize trauma-informed approaches and brief therapeutic techniques to support children and sponsors.
Maintain thorough documentation and safeguard confidential records, ensuring compliance with all program regulations.
Participate in ongoing case management, providing psychoeducation and additional services as needed.
Report any concerns related to abuse, neglect, or significant incidents to the appropriate authorities.
Travel to conduct in-person home visits and follow-ups (when required).
Participate in training sessions and contribute to quality improvement initiatives.

Minimum Qualifications
Bachelor's degree in Social Work (BSW) or a related field (e.g., psychology, sociology, behavioral sciences, or social services).
At least one year of experience in case management, specifically with immigrant populations.
Skilled in writing professional and comprehensive assessment reports.
Knowledge of child welfare practices, trauma-informed care, family preservation, kinship care, and relevant laws and regulations.
Strong communication and interpersonal skills to effectively interact with families, caseworkers, and other stakeholders.
Fluent in both English and Spanish (verbal/written).
Proficient in Microsoft Office and data entry systems.
Valid driver's license and willingness to travel for in-person visits when needed.
