Job Description

Who we are: La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As healthcare activists, we are dedicated to ensuring individuals who do not have health coverage receive the same level of quality healthcare as those who do. From our inception as a community health center in 1971, we understood that healing our communities requires addressing economic and social factors affecting health. This often involves providing services in various settings to link individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being. While we are known for our activism and spirit of social justice, we have also evolved into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa, and Solano counties.

Major Areas of Responsibility include but are not limited to:

Client Experience

  • Provide health education classes and group facilitation on various topics.
  • Effectively engage participants during health education workshops.
  • Assist with enrollment in health coverage and navigating the healthcare system.
  • Other duties as assigned by supervisor.

Commitment to Quality

  • Develop and maintain health education materials and resources.
  • Accurately document services and activities for program evaluation.
  • Contribute to grant reports and maintain program records.

Outreach and Advocacy

  • Train and coordinate peer health educators and Promotors for presentations and outreach.
  • Engage in community outreach to promote Health Center services.
  • Collaborate with community organizations and attend meetings.
  • Develop partnerships with community-based organizations.

Minimum Job Requirements

Knowledge

  • Knowledge of health education principles, methods, and materials.
  • Knowledge of relevant health education topics.
  • Familiarity with HIPAA compliance.
  • Strong communication skills.

Abilities

  • Ability to work independently and as part of a team.
  • Ability to work across cultures with a commitment to diversity and inclusion.
  • Maintain appropriate boundaries with clients.
  • Flexible schedule for classes and community events.
  • Bilingual in English and Spanish required.

Other Certifications and Experience

  • Current BLS certificate required.
  • Health Educator I: High school degree/GED, 1 year experience in health education in underserved communities, 2 years in a community setting.
  • Health Educator II: Bachelor's degree in relevant field, 2 years experience in health education in underserved communities.
  • Experience in peer counseling, community outreach, and group facilitation required.
